Address Section

I've been trying to change the order of the address section, but I don't know where to start. For example:

I would like to change:
123 Sesame St . . Phone:555-555-5555
Postal Code . . Fax: 555-555-5555
City . . Email: This email address is being protected from spambots. You need JavaScript enabled to view it.
Province/State
Country

and change it to this:

123 Sesame St. City, Province/State, Country, Postal Code
Phone:555-555-5555
Fax: 555-555-5555
Email: This email address is being protected from spambots. You need JavaScript enabled to view it.
